All fees are required to be paid before the service is rendered.

• Consultation with your Primary Care or Specialist Provider: $25 (billed by 15-minute increments)
• Consult Between Visits: $60 (20 minutes)
• Referral: $25 (billed by 15-minute increments)
• Lab/Imaging Review Visit Separate from Appointment: $35 (20 minutes)
• Medical Record Request: $50
• Prior Authorization: $25
• Medication refill outside of business hours: $25
• Forms/Letter Request: $100-$300 (based on time and complexity)
• Record Copying: $25
• QB Testing: $200
• GeneSight Testing: Tbd (covered by some insurances)
• Consultation for Legal Actions/Court Appearance: $300 hourly (6 hour minimum)
• Any additional services: $50-$150 for every 30-minute increment based on complexity
